Request for Proof of MAP Coverage
Upon request, Central Health will provide a proof of MAP coverage letter to the client making the request.
Staff Responsibilities:
- Verify all information in client record is correct (address and telephone number) for person requesting proof of coverage
- Inform client that they will receive their proof of coverage verification letter through the mail
- Submit contact “Request for Proof of Coverage”
Central Health Designated Staff Responsibilities:
- Send form letter to client with dates of MAP coverage.
