MAP Mail-In Applications can be submitted by mail to Central Health Eligibility Services at PO BOX 300489 Austin, TX 78703, or by fax to 512-901-9701.

Receiving and assigning mailed-In MAP applications

  1. When a mailed or faxed MAP application is received, each page of the application, the envelope and any supporting documents including a fax coversheet should be date-stamped with the date the application is received.
  2. Staple any multiple page documents together.
  3. Sort the documents and determine whether the application is complete which includes:
    1. All questions on the application are answered,
    2. The required supporting documents are included, and
    3. The application is signed and dated in the appropriate places.
  4. Separate the complete applications that have calculable income from the completed applications without income or with fixed income.
  5. Keep incomplete applications together regardless of income
  6. Sort applications as follows
    1. Completed applications with income – Eligibility Specialist
    2. Completed applications without income or with fixed income – Eligibility Associate
    3. Incomplete applications – Eligibility Specialist or Eligibility Associate
  7. Place the assigned applications in designated locations at the Central East Eligibility Office.

Processing mailed-in applications

Verify that the dates on all documents submitted with the mailed-in application are within 30 days of the post-marked date of the envelope (except those good for 45 days) and that the application is complete with all supporting documentation. If the documents are past the 30 or 45-day window or the application is otherwise incomplete, utilize the instructions found in the Medicaider/CHASSISTM User Manual, Chapter 5.I., Processing Mail-In Applications

  1. If the application is complete, utilize the instructions found in the Medicaider/CHASSISTM User Manual , Chapter 5.I., Processing Mail-In Applications.
  2. If a client is found eligible for MAP, enrollment duration should be determined as follows:
    • Currently enrolled – use day after current enrollment expires
    • New or expired coverage – use date of the last acceptable document received
  3. Determine the end date of coverage based on current MAP policies.
  4. An interview should be run for each person in the household and the certifications made accordingly.

Processing pended applications

  1. When documents from a pended application are received, date stamp each document received.Check to see if they were received by the deadline in the Missing Document Request letter or before the signature on the Applicant Responsibilities on the original mailed-in applications expires which is 30 days. If the application is still incomplete, process the application as a denial.
  2. Staple any multiple page documents together.
  3. Follow the above instructions in Section 1., Receiving and assigning mailed-In MAP applications.
  4. Utilize the instructions found in the Medicaider/CHASSISTM User Manual , Chapter 5.J., Processing Pended Document for Mail-In Applications, to process the pended application in Medicaider/CHASSISTM
