Family Planning Program

The Family Planning Program provides comprehensive family planning services to reduce unintended pregnancies, positively affect future pregnancies, and improve health status of women and men.

For eligibility criteria and application instructions, please refer to:

  • The Family Planning Services (FPS) outline
  • The HTW and Family Planning Screening Chart
  • How to complete TX Family Planning Form 11/7/2019
    • Additional Notes:
      • The email field in the Communication Preferences field must be completed by hand if it is cut off
      • Do not leave any field blank
      • For the Race/Ethnicity field enter the  complete race and the ethnicity. Please spell out White/Anglo, White/Hispanic, Black/African American, Asian/Asian American etc.
      • The name under the income section auto-populates in the First, Middle, Last Name format, this is okay, you do not have to change it to Last, Middle, First format.
      • For the income section, if the household received $0 income, the forms does not auto-populated $0. Enter the name of the client applying for the grant and $0 as the amount regardless of who signs the zero income.

Notes for MAP Staff:

  • When certifying for the Family Planning Program  grant the form must be scanned in the enrolled client’s Family Planning Program track under “Additional Supportive Documents”.
  • All other forms (HTW Screening tool, HTW application) must be scanned in the Head of Household’s MAP or CCHC track under “CH All Other Proof Documents”.

Global Maternity 

The Global Maternity Program provides a discount on prenatal healthcare related services (office visits, laboratory services, and ultrasounds) to pregnant women.

Notes for MAP Staff

  • All pregnant women MAP/CCHC enrolled clients who are established at CommUnityCare are eligible to receive this benefit.
  • Enrollment fee is a one-time charge and is determined by income and household size.
