Obtaining Medical, Dental or Urgent Care Services

Established patient– seen at PCP/health center location in last 18 months

Unestablished patient– never seen or not seen in the last 18 months at PCP/health center location

OBTAINING MAP MEDICAL SERVICES:

CommUnityCare  Health Center Patient-Unestablished:

  • If the patient indicates they need a non-emergency  or non-urgent care medical appointment, instruct the patient to call the Patient Navigation Center at 512-978-9015 to schedule a medical appointment.
  • If the patient indicates they have an urgent care medical need, follow the urgent care instructions below for MAP patients.

If patient is established at a health center, instruct them to call their assigned Health Center to obtain a medical appointment

CommUnityCare ARCH patient- Established or Unestablished

  • Instruct the patient to go to the following walk-in clinic location: Austin Resource Center for the Homeless (ARCH) -500 East 7th Street 512-978-9920

OBTAINING MAP URGENT CARE SERVICES:

(Non-life threatening emergency that requires care within 24 hours)

Established or Unestablished Patients (PCP does not matter):

  • Instruct the patient to:
    • Walk-in to one of the Urgent or Convenient Care clinics, or
    • Contact their assigned PCP directly and attempt to schedule a medical appointment, or
    • If PCP is CommUnityCare, they may be directed to walk in to either the CommUnityCare Hancock or Southeast Health and Wellness Center.

OBTAINING DENTAL SERVICES:

    • MAP provides full covered dental benefits including dentures
    • Instruct the patient to call one of the dental clinics directly
    • Patients with active Medicare, Adult Medicaid or CHIP and established at a CommUnityCare Health Center may utilize the CommUnityCare dental clinics
    • MAP enrollees have access to CommUnityCare dental clinics regardless of assigned PCP
