Identification Policy

Policy: To be considered eligible for the Central Health Medical Access Program and to prevent fraud, Travis County residents must verify their identity.

Identification Requirements:

  1. The following household members must provide an acceptable form of photo identification as proof of identity.
    1.  Adult family members
    2. Emancipated minors
    3. A minor child living alone or with others who are not responsible for the child’s support.
  2.  For the purpose of this policy, the photo identification:
    1. Does not need to be current
    2. Must have the name of the applicant
  3. The following documents are acceptable forms of identification if the documents contains a photo:
    1. Driver’s license/State identification card/Temporary identification
    2. Passport
    3. Consulate identification card
    4. Foreign governmental identification
    5. USCIS document (e.g., Legal Permanent Resident Card, Employment Authorization Card, Arrival/Departure Forms, etc.)
    6. School or employment identification
    7. Any business or agency identification card (e.g. Capital Metro, Sam’s Warehouse, ServicePoint, Salvation Army, Money Box Store)
  4. The following documents without a photo may be used for a period of one year:
    1. Birth Certificate and/or Identification Form Statement
    2. Birth certificate
    3. Letter from an agency with agency name, representative name, phone number and date
    4. Marriage license
    5. Printout from the Social Security Administration
    6. School records
    7. Social Security card
    8. Temporary driver’s license
    9. Voter’s registration card
    10. Child’s birth certificate (or other document, which identifies the client as the parent)

      Note: At the client’s next visit, one of the required documents listed in Section III must be presented to a Program Representative. An exception may be used again if client has not had MAP coverage for 5 years.

  5. If a client is unable to obtain an acceptable form of photo identification, after the one year period one of the following documents may be accepted on an ongoing basis upon supervisor approval
    1. A Birth Certificate and/or Identification Form Statement that reflects the client’s name, date of birth and parent(s) name.
    2. Child’s birth certificate (or other document, which identifies the client as the parent)
  6. Procedures
    1. Collect documents
    2. If client is new, scan documents into the CH All Identity Documents folder,
    3. If client is renewing:
      • If household remains the same, carry over all valid documents for current household members into new certification under CH All Identify Documents,
      • If there is a change in household composition, print and scan ONLY the documents that apply to existing household
    4. Inform client of any exceptions made.
